Narrative:
We were assigned the standard SID which involved tracking a radial from clt VOR. After departure we were given runway heading to intercept the radial for the SID. Climbing on runway heading through approximately 4000 ft departure control asked if we were showing ourselves on course. We answered that the assigned radial was to the left of our track and we had not intercepted the assigned radial yet. Departure informed us that in fact the assigned radial was to our right and we were 4 NM east of course and proceeding toward the arrival gate. I double checked the radial and VOR which were set right. Departure assigned us a heading for departure while we trouble-shot our navigational equipment. It was then I discovered that the previous captain had set the VOR/ILS alternate switch to the first officer's position. This meant my navigation was receiving information from the first officer's navigation, which was selected to a downline VOR. The switch was returned to normal position and all navigation receivers indicated normal. Climb out then proceeded as normal. During the originating checklist, I remember calling 'checked and set' when transfer switches were called, but obviously did not physically check the switch position. This occurred at the end of another long duty day and I believe fatigue played a factor.

Title: FLC TECHNIQUE PREFLT PROC ACR READ, BUT DID NOT ACCOMPLISH THEIR PREFLT CHKLIST.
Narrative: WE WERE ASSIGNED THE STANDARD SID WHICH INVOLVED TRACKING A RADIAL FROM CLT VOR. AFTER DEP WE WERE GIVEN RWY HDG TO INTERCEPT THE RADIAL FOR THE SID. CLBING ON RWY HDG THROUGH APPROX 4000 FT DEP CTL ASKED IF WE WERE SHOWING OURSELVES ON COURSE. WE ANSWERED THAT THE ASSIGNED RADIAL WAS TO THE L OF OUR TRACK AND WE HAD NOT INTERCEPTED THE ASSIGNED RADIAL YET. DEP INFORMED US THAT IN FACT THE ASSIGNED RADIAL WAS TO OUR R AND WE WERE 4 NM E OF COURSE AND PROCEEDING TOWARD THE ARR GATE. I DOUBLE CHKED THE RADIAL AND VOR WHICH WERE SET RIGHT. DEP ASSIGNED US A HDG FOR DEP WHILE WE TROUBLE-SHOT OUR NAVIGATIONAL EQUIP. IT WAS THEN I DISCOVERED THAT THE PREVIOUS CAPT HAD SET THE VOR/ILS ALTERNATE SWITCH TO THE FO'S POS. THIS MEANT MY NAV WAS RECEIVING INFO FROM THE FO'S NAV, WHICH WAS SELECTED TO A DOWNLINE VOR. THE SWITCH WAS RETURNED TO NORMAL POS AND ALL NAV RECEIVERS INDICATED NORMAL. CLBOUT THEN PROCEEDED AS NORMAL. DURING THE ORIGINATING CHKLIST, I REMEMBER CALLING 'CHKED AND SET' WHEN TRANSFER SWITCHES WERE CALLED, BUT OBVIOUSLY DID NOT PHYSICALLY CHK THE SWITCH POS. THIS OCCURRED AT THE END OF ANOTHER LONG DUTY DAY AND I BELIEVE FATIGUE PLAYED A FACTOR.
