Narrative:

Pushback at ord was normal. When the push was complete I told the ground's person that he was cleared to disconnect. The first officer called for taxi and said we were cleared out after the 767 about 200 ft further out the alley. I cleared the area from in front of the first officer to the extreme left and announced clear left. The first officer cleared the area and announced clear right. I had just started to roll when I realized I had not seen the pin or received the salute. I immediately stopped the aircraft. Aircraft movement had been 1-2 ft. With the aircraft now stopped, 1 of the ground's people came running out and held up crossed arms. Until he came out we could not see the tug or any ground's personnel from either seat. Normal procedures were not followed by announcing that we were cleared to taxi and also by me listening to ramp prior to receiving the salute. But also, positioning the tug a little further from the aircraft while the towbar was being disconnected and/or having a guideman in front of the aircraft giving the stop hand signal as is done many times would have prevented us from clearing the area. Of course the error is ours and we had heard of others doing this but never thought it would happen to me. It did. Fortunately, there was no contact with equipment or personnel and we both learned from the occurrence.

THE FO CALLED FOR TAXI AND SAID WE WERE CLRED OUT AFTER THE 767 ABOUT 200 FT FURTHER OUT THE ALLEY. I CLRED THE AREA FROM IN FRONT OF THE FO TO THE EXTREME L AND ANNOUNCED CLR L. THE FO CLRED THE AREA AND ANNOUNCED CLR R. I HAD JUST STARTED TO ROLL WHEN I REALIZED I HAD NOT SEEN THE PIN OR RECEIVED THE SALUTE. I IMMEDIATELY STOPPED THE ACFT. ACFT MOVEMENT HAD BEEN 1-2 FT. WITH THE ACFT NOW STOPPED, 1 OF THE GND'S PEOPLE CAME RUNNING OUT AND HELD UP CROSSED ARMS. UNTIL HE CAME OUT WE COULD NOT SEE THE TUG OR ANY GND'S PERSONNEL FROM EITHER SEAT. NORMAL PROCEDURES WERE NOT FOLLOWED BY ANNOUNCING THAT WE WERE CLRED TO TAXI AND ALSO BY ME LISTENING TO RAMP PRIOR TO RECEIVING THE SALUTE. BUT ALSO, POSITIONING THE TUG A LITTLE FURTHER FROM THE ACFT WHILE THE TOWBAR WAS BEING DISCONNECTED AND/OR HAVING A GUIDEMAN IN FRONT OF THE ACFT GIVING THE STOP HAND SIGNAL AS IS DONE MANY TIMES WOULD HAVE PREVENTED US FROM CLRING THE AREA.
