Primary Problem | Flight Crew Human Performance |
Air Traffic Incident | Pilot Deviation |
Narrative:
The problem was a miscom between crew. We were instructed by tower to allow commuter aircraft to pass right to left and contact ground. We allowed the commuter to pass and then taxied to gate. While en route (short distance) there was a question concerning clearance to gate by tower or contact ground. Called ground and were advised of no problem continue taxi to gate. No conflicts. I believe primary cause was scheduled reduced rest of 8 hours previous evening and compromised crew communication. Problem was discovered by proper crew rescue management and corrected promptly. Crew believes primary factor in mistake was a result of minimum sleep. Recommend minimum 9 hours reduced rest and worded such so that is not scheduled reduced rest. Crew recommends for present to take whatever extra time necessary to minimize human factor mistakes.

Title: FLC DID NOT COMPLY WITH CTLR INSTRUCTION TO CONTACT GND.
Narrative: THE PROB WAS A MISCOM BTWN CREW. WE WERE INSTRUCTED BY TWR TO ALLOW COMMUTER ACFT TO PASS R TO L AND CONTACT GND. WE ALLOWED THE COMMUTER TO PASS AND THEN TAXIED TO GATE. WHILE ENRTE (SHORT DISTANCE) THERE WAS A QUESTION CONCERNING CLRNC TO GATE BY TWR OR CONTACT GND. CALLED GND AND WERE ADVISED OF NO PROB CONTINUE TAXI TO GATE. NO CONFLICTS. I BELIEVE PRIMARY CAUSE WAS SCHEDULED REDUCED REST OF 8 HRS PREVIOUS EVENING AND COMPROMISED CREW COM. PROB WAS DISCOVERED BY PROPER CREW RESCUE MGMNT AND CORRECTED PROMPTLY. CREW BELIEVES PRIMARY FACTOR IN MISTAKE WAS A RESULT OF MINIMUM SLEEP. RECOMMEND MINIMUM 9 HRS REDUCED REST AND WORDED SUCH SO THAT IS NOT SCHEDULED REDUCED REST. CREW RECOMMENDS FOR PRESENT TO TAKE WHATEVER EXTRA TIME NECESSARY TO MINIMIZE HUMAN FACTOR MISTAKES.
