Narrative:
Daily service maintenance check expired prior to flight from fll-pie- stl. Neither flight crew nor maintenance personnel noted that daily service check expired prior to flight. Extenuating circumstances: aircraft change to aircraft with expired daily service check during short ground time after aforementioned aircraft arrived late. Aircraft arrived with deferred maintenance that required extra time to review all MEL requirements and comply with specific procedures. En route WX necessitated additional time to evaluate for flight plan purposes. Company maintenance personnel were occupied with maintenance problem on another aircraft. Recommendations: insure adequate ground time for all required operational and maintenance actions to be completed without being compromised by compressed ground time due to WX, flight plan or maintenance factors. De-emphasize and change delay reporting criteria to allow for extra ground necessitated by late arriving inbound flts. Continue to research the human factor involved in the 'hurry up mode' and provide appropriate relief for delay reporting.
Original NASA ASRS Text
Title: LGT OPERATED BEYOND THE ACFT DAILY INSPECTION LIMITS.
Narrative: DAILY SVC MAINT CHK EXPIRED PRIOR TO FLT FROM FLL-PIE- STL. NEITHER FLC NOR MAINT PERSONNEL NOTED THAT DAILY SVC CHK EXPIRED PRIOR TO FLT. EXTENUATING CIRCUMSTANCES: ACFT CHANGE TO ACFT WITH EXPIRED DAILY SVC CHK DURING SHORT GND TIME AFTER AFOREMENTIONED ACFT ARRIVED LATE. ACFT ARRIVED WITH DEFERRED MAINT THAT REQUIRED EXTRA TIME TO REVIEW ALL MEL REQUIREMENTS AND COMPLY WITH SPECIFIC PROCS. ENRTE WX NECESSITATED ADDITIONAL TIME TO EVALUATE FOR FLT PLAN PURPOSES. COMPANY MAINT PERSONNEL WERE OCCUPIED WITH MAINT PROB ON ANOTHER ACFT. RECOMMENDATIONS: INSURE ADEQUATE GND TIME FOR ALL REQUIRED OPERATIONAL AND MAINT ACTIONS TO BE COMPLETED WITHOUT BEING COMPROMISED BY COMPRESSED GND TIME DUE TO WX, FLT PLAN OR MAINT FACTORS. DE-EMPHASIZE AND CHANGE DELAY RPTING CRITERIA TO ALLOW FOR EXTRA GND NECESSITATED BY LATE ARRIVING INBOUND FLTS. CONTINUE TO RESEARCH THE HUMAN FACTOR INVOLVED IN THE 'HURRY UP MODE' AND PROVIDE APPROPRIATE RELIEF FOR DELAY RPTING.
