Narrative:
We took over aircraft from another crew. Aircraft was late arriving from dfw because of last min #4 tire change. Our takeoff appeared normal, except for momentary anti-skid caution, which is not uncommon on 'a' model saab 340. Within 10 mins after takeoff, our company in temple called us on radio and said observers witnessed a wheel fall off our plane on takeoff. We continued flight to our destination, dfw. After coordinating with our dispatcher and ATC, we did a low fly-by at dfw. It was confirmed by dfw tower and aircraft on ground that our #4 wheel (right outboard) was missing, however #3 wheel was there with normal 3 green and locked indications, we landed on runway 13L, dfw, uneventfully. As a precaution, emergency had been declared, crash fire rescue equipment was standing by. Ground evacuate/evacuation was planned, if needed. We were told, later, by company that the #4 tire change was performed incorrectly resulting in loss of wheel. Appears to be a mechanical quality-control problem.

Title: ACFT EQUIP PROB CRITICAL MAIN WHEEL FELL OFF ON TKOF.
Narrative: WE TOOK OVER ACFT FROM ANOTHER CREW. ACFT WAS LATE ARRIVING FROM DFW BECAUSE OF LAST MIN #4 TIRE CHANGE. OUR TKOF APPEARED NORMAL, EXCEPT FOR MOMENTARY ANTI-SKID CAUTION, WHICH IS NOT UNCOMMON ON 'A' MODEL SAAB 340. WITHIN 10 MINS AFTER TKOF, OUR COMPANY IN TEMPLE CALLED US ON RADIO AND SAID OBSERVERS WITNESSED A WHEEL FALL OFF OUR PLANE ON TKOF. WE CONTINUED FLT TO OUR DEST, DFW. AFTER COORDINATING WITH OUR DISPATCHER AND ATC, WE DID A LOW FLY-BY AT DFW. IT WAS CONFIRMED BY DFW TWR AND ACFT ON GND THAT OUR #4 WHEEL (R OUTBOARD) WAS MISSING, HOWEVER #3 WHEEL WAS THERE WITH NORMAL 3 GREEN AND LOCKED INDICATIONS, WE LANDED ON RWY 13L, DFW, UNEVENTFULLY. AS A PRECAUTION, EMER HAD BEEN DECLARED, CFR WAS STANDING BY. GND EVAC WAS PLANNED, IF NEEDED. WE WERE TOLD, LATER, BY COMPANY THAT THE #4 TIRE CHANGE WAS PERFORMED INCORRECTLY RESULTING IN LOSS OF WHEEL. APPEARS TO BE A MECHANICAL QUALITY-CTL PROB.
