Narrative:
Small aircraft X0C was cleared for approach to lyh airport. Immediately thereafter I heard 'zero charlie has airport -- canceled IFR.' I removed X0C from system only to find out later it was another 0C that canceled IFR at another airport (hsp). Shortly after the removal of X0C -- a C182 was released at lyh and loss of separation occurred with X0C who advised tower he never canceled IFR.

Title: SMA X ON APCH HAD LTSS FROM C182 DEP. SYS ERROR.
Narrative: SMA X0C WAS CLRED FOR APCH TO LYH ARPT. IMMEDIATELY THEREAFTER I HEARD 'ZERO CHARLIE HAS ARPT -- CANCELED IFR.' I REMOVED X0C FROM SYS ONLY TO FIND OUT LATER IT WAS ANOTHER 0C THAT CANCELED IFR AT ANOTHER ARPT (HSP). SHORTLY AFTER THE REMOVAL OF X0C -- A C182 WAS RELEASED AT LYH AND LOSS OF SEPARATION OCCURRED WITH X0C WHO ADVISED TWR HE NEVER CANCELED IFR.
