Narrative:
Flight ord-slc, was dispatched with inoperative #1 fuel quantity gauge, per MEL. After flaps up, crew noticed 3 full units left rudder trim required to hold wings level. When rudder trim was zeroed, required 3 full units left aileron trim for wings level. Suspect 3000-4000 pound fuel imbalance between left and right tanks (#1 and #3). Advised dispatch landing short at dia (den) instead slc, account observed high fuel burnout, and suspected fuel loading far out of balance limits, potential for flameout, #1 engine. Approach and landing accomplished with excessive trim required, but no incident. Followed published sops and notifications.

Title: INOP FUEL GAUGE. ACFT MIS-FUELED WITH THE #1 FUEL GAUGE INOP, THE FLC HAD NOT CAUGHT THE FUELING.
Narrative: FLT ORD-SLC, WAS DISPATCHED WITH INOP #1 FUEL QUANTITY GAUGE, PER MEL. AFTER FLAPS UP, CREW NOTICED 3 FULL UNITS L RUDDER TRIM REQUIRED TO HOLD WINGS LEVEL. WHEN RUDDER TRIM WAS ZEROED, REQUIRED 3 FULL UNITS L AILERON TRIM FOR WINGS LEVEL. SUSPECT 3000-4000 LB FUEL IMBALANCE BTWN L AND R TANKS (#1 AND #3).
