Narrative:
During preflight the INS (2 units) and omega were loaded with the waypoints to fargo, nd. Crew did not go to 'align' with the INS's so at start check the captain noticed that the INS's were not aligned and decided to position the INS selectors to altitude reference which shuts down the INS and said 'we're going with the omega only.' en route to far, approximately 160 mi out of panama city, panama, the omega became unreliable for navigation and captain used dead reckoning to navigation to gni. According to operations specifications the DC10 is required to use 2 INS's or 1 INS and 1 omega to fly across the caribbean and the captain did not want to wait in panama city, panama, for the INS to align and delay the departure.

Title: A SUPPLEMENTAL CARRIER DC10 FLC DEPARTED ON A FLT ACROSS THE CARIBBEAN WITHOUT THE MINIMUM REQUIRED NAV EQUIP OPERATING. THE FLC HAD FORGOTTEN TO INITIALIZE THE INS ALIGN CHK DURING THE PREFLT, AND CHOSE NOT TO DELAY THE FLT'S DEP.
Narrative: DURING PREFLT THE INS (2 UNITS) AND OMEGA WERE LOADED WITH THE WAYPOINTS TO FARGO, ND.
