Narrative:
In sum: flight crew entered flight crew plan: slc-mlf flight plan route in the FMS. They forgot to enter the mlf 2 SID and therefore flew out the 183 degree radial direct to mlf vice the 193 degree radial depicted by the SID.

Title: LGT FAILED TO FLY SID RADIALS FROM SLC.
Narrative: IN SUM: FLC ENTERED FLC PLAN: SLC-MLF FLT PLAN RTE IN THE FMS. THEY FORGOT TO ENTER THE MLF 2 SID AND THEREFORE FLEW OUT THE 183 DEG RADIAL DIRECT TO MLF VICE THE 193 DEG RADIAL DEPICTED BY THE SID.
