Narrative:

Lead flight attendant did not relock cockpit door after entry. Following her exit, passenger mistakenly opened cockpit door. Relocked door and briefed flight attendant about locking door. This problem is caused by manufacturer's design of A320 cockpit door lock. Unlike other transport aircraft, this door does not always remain locked, and must be relocked after entry via key.
