Narrative:

Departure climbed to 15000 ft, handed off to center, frequency change accomplished. Overflt inbound at 15000 ft, handed off to TRACON, no frequency change. Center controller did not respond to the first 2 calls of the departure, did not have positive radar identify, and did not climb the departure to requested altitude, nor did he put the overflt on TRACON frequency. TRACON controller assumed center would climb the departure and did not put a restr on the overflt before taking handoff. TRACON controller saw situation and attempted corrective action on guard frequency.

Title: TRACON CTLR HANDED OFF AN IFR MIL ACFT AT 15000 FT TO ZFW AND ACCOMPLISHED THE FREQ CHANGE. AT THE SAME TIME AN IFR MIL OVERFLT WAS HANDED OFF TO THE RPTR LEVEL AT 15000 FT. RPTRS ATTEMPTS TO ALERT THE CTR CTLR WERE NOT SUCCESSFUL RESULTING IN LTSS.

Narrative: DEP CLBED TO 15000 FT, HANDED OFF TO CTR, FREQ CHANGE ACCOMPLISHED. OVERFLT INBOUND AT 15000 FT, HANDED OFF TO TRACON, NO FREQ CHANGE. CTR CTLR DID NOT RESPOND TO THE FIRST 2 CALLS OF THE DEP, DID NOT HAVE POSITIVE RADAR IDENT, AND DID NOT CLB THE DEP TO REQUESTED ALT, NOR DID HE PUT THE OVERFLT ON TRACON FREQ. TRACON CTLR ASSUMED CTR WOULD CLB THE DEP AND DID NOT PUT A RESTR ON THE OVERFLT BEFORE TAKING HDOF. TRACON CTLR SAW SIT AND ATTEMPTED CORRECTIVE ACTION ON GUARD FREQ.
