Narrative:

At the beginning of our day, we received information from the computer network that we would fly aircraft a from hts to pit to bgm to pit, at which time we would swap into aircraft B. A change was made requiring us to swap aircraft in bgm instead of pit. We were not made aware of this change with the exception of a different tail number on our release. The release was delivered to the aircraft late in the boarding process during a busy preparation time for the flight crew. The inaccurate data on the release was unnoticed by the flight crew, and the flight departed in this condition. Corrective actions could include better communications between dispatch and flcs as well as increased awareness by the flight crew when reviewing the release form.

Title: MDT WAS SCHEDULED TO FLY HTS TO PIT TO BGM AND RETURN TO PIT WHERE A PLANE CHANGE WAS TO TAKE PLACE. THIS PLAN WAS CHANGED. THE PLANE CHANGE WAS NOW TO BE MADE IN BGM. THE ONLY NOTICE OF THIS WAS A DIFFERENT TAIL NUMBER IN THE NEW RELEASE WHICH THE CREW DID NOT NOTICE. THEREFORE, THE WRONG ACFT WAS FLOWN FROM BGM TO PIT. FLC SUGGESTS A MORE OBVIOUS MEANS BE USED TO ADVISE CREWS OF THIS SORT OF CHANGE.

Narrative: AT THE BEGINNING OF OUR DAY, WE RECEIVED INFO FROM THE COMPUTER NETWORK THAT WE WOULD FLY ACFT A FROM HTS TO PIT TO BGM TO PIT, AT WHICH TIME WE WOULD SWAP INTO ACFT B. A CHANGE WAS MADE REQUIRING US TO SWAP ACFT IN BGM INSTEAD OF PIT. WE WERE NOT MADE AWARE OF THIS CHANGE WITH THE EXCEPTION OF A DIFFERENT TAIL NUMBER ON OUR RELEASE. THE RELEASE WAS DELIVERED TO THE ACFT LATE IN THE BOARDING PROCESS DURING A BUSY PREPARATION TIME FOR THE FLC. THE INACCURATE DATA ON THE RELEASE WAS UNNOTICED BY THE FLC, AND THE FLT DEPARTED IN THIS CONDITION. CORRECTIVE ACTIONS COULD INCLUDE BETTER COMS BTWN DISPATCH AND FLCS AS WELL AS INCREASED AWARENESS BY THE FLC WHEN REVIEWING THE RELEASE FORM.
