Narrative:
I left ZZZ on an air ambulance flight to ZZZ at AB38 and arrived at AE50. Departed ZZZ at AH10 en route to ZZZ for customs. Arrived in ZZZ at AN40 and departed for ZZZ at AP40. I believe the problem started at departure from ZZZ when I couldn't get my IFR clearance on the ground. I departed ZZZ VFR and tried to raise ZZZ approach and was not able to contact them for about 5 mins. During this time I was distracted by the nurses in the back of plane and the passenger with the medical problems making lots of noise. By the time I got hold of ZZZ approach and had gotten into ZZZ airspace, according to ZZZ approach. I was given my clearance to ZZZ and was then informed by ZZZ approach that I was 2 mi from ZZZ and that a S3 aircraft had to take evasive action that was in the ZZZ pattern. I believe the factors that led up to this event was the amount of hours I had already flown this day. I was tired and distracted by passenger and nurses and was pushed by my company to finish the mission.

Narrative: I LEFT ZZZ ON AN AIR AMBULANCE FLT TO ZZZ AT AB38 AND ARRIVED AT AE50. DEPARTED ZZZ AT AH10 ENRTE TO ZZZ FOR CUSTOMS. ARRIVED IN ZZZ AT AN40 AND DEPARTED FOR ZZZ AT AP40. I BELIEVE THE PROB STARTED AT DEP FROM ZZZ WHEN I COULDN'T GET MY IFR CLRNC ON THE GND. I DEPARTED ZZZ VFR AND TRIED TO RAISE ZZZ APCH AND WAS NOT ABLE TO CONTACT THEM FOR ABOUT 5 MINS. DURING THIS TIME I WAS DISTRACTED BY THE NURSES IN THE BACK OF PLANE AND THE PAX WITH THE MEDICAL PROBS MAKING LOTS OF NOISE.
