Narrative:

On apr/tue/97 I received an EMS flight request at about XX00 hours. I departed X helipad heading northbound. Prior to departing, I contacted aa tower, was given a squawk code and told to proceed. After crossing, aa tower said to contact B approach. Called B approach. At this time I thought I was under B control. As I was crossing runway Z at airport C, a jet was coming in on final. I descended to remain clear. After this, approximately 5 mins later, I was contacted by B approach control and advised that I needed to let them know if I was going to take the beach or shoreline departure route. My assumption was I was under B control and I did not contact C tower.

Title: AN EMS HELI RPTR PLT CONTACTED B TRACON CTLR AS INSTRUCTED AND RPTR THEN ASSUMED HE WAS UNDER POSITIVE CTL OF THE DEP CTLR. AFTER XING ANOTHER ARPT ENRTE HE HAD TO TAKE EVASIVE ACTION TO AVOID AN ACFT ON APCH. DEP CTLR LATER ADVISED RPTR TO LET THEM KNOW IN THE FUTURE WHAT RTE HE WAS PLANNING TO FLY.

AFTER XING, AA TWR SAID TO CONTACT B APCH. CALLED B APCH. AT THIS TIME I THOUGHT I WAS UNDER B CTL. AS I WAS XING RWY Z AT ARPT C, A JET WAS COMING IN ON FINAL. I DSNDED TO REMAIN CLR. AFTER THIS, APPROX 5 MINS LATER, I WAS CONTACTED BY B APCH CTL AND ADVISED THAT I NEEDED TO LET THEM KNOW IF I WAS GOING TO TAKE THE BEACH OR SHORELINE DEP RTE. MY ASSUMPTION WAS I WAS UNDER B CTL AND I DID NOT CONTACT C TWR.
