Narrative:
Due to a mechanical problem with the fueling truck, fueling the aircraft at the gate in sfo began very late. At scheduled departure time, fueling continued and shortly thereafter, the agent closed the forward entry door. Both cockpit crew members were preoccupied with flight deck duties and were temporarily unaware of the part 1 deviation. Fueling was concluded before any action could be taken.

Title: THE FLC OF AN ACR B767 DID NOT REALIZE THAT REFUELING WAS STILL IN PROGRESS WHEN THE BOARDING AGENT SHUT THE DOOR AND RETRACTED THE JETWAY.
Narrative: DUE TO A MECHANICAL PROB WITH THE FUELING TRUCK, FUELING THE ACFT AT THE GATE IN SFO BEGAN VERY LATE. AT SCHEDULED DEP TIME, FUELING CONTINUED AND SHORTLY THEREAFTER, THE AGENT CLOSED THE FORWARD ENTRY DOOR. BOTH COCKPIT CREW MEMBERS WERE PREOCCUPIED WITH FLT DECK DUTIES AND WERE TEMPORARILY UNAWARE OF THE PART 1 DEV. FUELING WAS CONCLUDED BEFORE ANY ACTION COULD BE TAKEN.
