Narrative:
We were on the last leg of day 3 of a 4-DAY trip. I now realize that I was coming down with a cold, and I was quite fatigued and had been sleeping poorly. We were flying the drake 6 SID, which calls for runway heading to 9 mi, at which time a right turn to a 360 degree heading is performed in order to join the pxr 336 degree radial. I was in the process of looking down to rearrange some charts, and when I looked up again we were at 12+ mi from the VOR and still on runway heading. I told the first officer to turn now and to make it tight! Departure said nothing, but after we had rolled out on the 360 degree heading, they vectored us further east to join the 336 degree radial. The first officer and PF was a 50 yr old subpar performer who came aboard when we acquired a smaller carrier. Having to watch him like a hawk is another reason why I was so fatigued after 3 days of flying. As you can see, looking away for a short period on a simple SID is not even advisable with this guy. If the airline is smart, he won't make it through upgrade, but then again, they shouldn't have let him get off probation after we acquired his sorry airline. He's what I call a 'CDU geek.' he loves to enter data into the CDU, but can't fly the plane to save his life! He is also resentful of the fact that the capts he flies with are generally younger than him and have already spent more time with a successful airline than he ever will.
Original NASA ASRS Text
Title: FLC OF AN MLG FAILED TO TURN AS REQUIRED DURING A DEP SID. AFTER RPTR DISCOVERED ERROR AND HAD FO TURNING ACFT TO THE REQUIRED HDG, CTLR ISSUED A HDG CHANGE TO BRING THE ACFT BACK ON ORIGINAL TRACK.
