Narrative:

Location: sna. Flight xx sna-dfw jul/ab/97. B757. This report is submitted in conjunction with a previous report submitted on jul/xx/97 for flight xyz dfw to lax jul/97. The scheduled departure time for flight xx was AA50 PDT. The passenger were still boarding at AA47 PDT. A gate agent entered cockpit and explained that they did not have room in the cabin for some oxygen bottles so they were going to place them in the forward cargo bay. He provided me with a form that indicated there would be 2 oxygen bottles in the forward cargo bay. Before he was able to leave the aircraft another agent began to close the forward entry door. The forward entry door had to be reopened to let the first agent off the aircraft. The forward entry door was closed at AA49. There were still passenger standing. The radio closeout was received but did not show the oxygen bottle hazmat. We received clearance to push at AA51. We called sna operations via radio at AA51 concerning the lack of RA's on the radio closeout. We started our engines and called operations again at AA56 inquiring about our closeout. We taxied at BB01 to a position out of the way of the flow of taxiing aircraft. At BB03 we got a message from operations that an accurate closeout was sent. We continued to talk to operations regarding the lack of proper closeout until BB08 when an accurate closeout was received. We would not have received a proper closeout for the oxygen bottle hazmat if we had not been persistent about getting one as required. It is apparent that the pressures of on-time departures are affecting attention to detail -- regulatory and procedural compliance, and safety. The takeoff was actually delayed 12 mins to attain a proper closeout as required. Copies of all pertinent documentation are available.

Title: A B757 TAXIES OUT AT SNA, CA, WITHOUT PROPER PAPERWORK FOR OXYGEN BOTTLES STORED IN CARGO COMPARTMENT. FLC PERSISTENTLY REQUEST PROPER PAPERWORK AND DEPART LATE.

Narrative: LOCATION: SNA. FLT XX SNA-DFW JUL/AB/97. B757. THIS RPT IS SUBMITTED IN CONJUNCTION WITH A PREVIOUS RPT SUBMITTED ON JUL/XX/97 FOR FLT XYZ DFW TO LAX JUL/97. THE SCHEDULED DEP TIME FOR FLT XX WAS AA50 PDT. THE PAX WERE STILL BOARDING AT AA47 PDT. A GATE AGENT ENTERED COCKPIT AND EXPLAINED THAT THEY DID NOT HAVE ROOM IN THE CABIN FOR SOME OXYGEN BOTTLES SO THEY WERE GOING TO PLACE THEM IN THE FORWARD CARGO BAY. HE PROVIDED ME WITH A FORM THAT INDICATED THERE WOULD BE 2 OXYGEN BOTTLES IN THE FORWARD CARGO BAY. BEFORE HE WAS ABLE TO LEAVE THE ACFT ANOTHER AGENT BEGAN TO CLOSE THE FORWARD ENTRY DOOR. THE FORWARD ENTRY DOOR HAD TO BE REOPENED TO LET THE FIRST AGENT OFF THE ACFT. THE FORWARD ENTRY DOOR WAS CLOSED AT AA49. THERE WERE STILL PAX STANDING. THE RADIO CLOSEOUT WAS RECEIVED BUT DID NOT SHOW THE OXYGEN BOTTLE HAZMAT. WE RECEIVED CLRNC TO PUSH AT AA51. WE CALLED SNA OPS VIA RADIO AT AA51 CONCERNING THE LACK OF RA'S ON THE RADIO CLOSEOUT. WE STARTED OUR ENGS AND CALLED OPS AGAIN AT AA56 INQUIRING ABOUT OUR CLOSEOUT. WE TAXIED AT BB01 TO A POS OUT OF THE WAY OF THE FLOW OF TAXIING ACFT. AT BB03 WE GOT A MESSAGE FROM OPS THAT AN ACCURATE CLOSEOUT WAS SENT. WE CONTINUED TO TALK TO OPS REGARDING THE LACK OF PROPER CLOSEOUT UNTIL BB08 WHEN AN ACCURATE CLOSEOUT WAS RECEIVED. WE WOULD NOT HAVE RECEIVED A PROPER CLOSEOUT FOR THE OXYGEN BOTTLE HAZMAT IF WE HAD NOT BEEN PERSISTENT ABOUT GETTING ONE AS REQUIRED. IT IS APPARENT THAT THE PRESSURES OF ON-TIME DEPS ARE AFFECTING ATTN TO DETAIL -- REGULATORY AND PROCEDURAL COMPLIANCE, AND SAFETY. THE TKOF WAS ACTUALLY DELAYED 12 MINS TO ATTAIN A PROPER CLOSEOUT AS REQUIRED. COPIES OF ALL PERTINENT DOCUMENTATION ARE AVAILABLE.
