Narrative:
During preflight checks we noticed an inoperative loop 'a' on the #2 engine. Maintenance was called out and the item was put on MEL. Although dispatch was notified by maintenance control of the addition of the MEL, there was no communication between the flight deck and dispatch. Therefore the appropriate notation was never added to the flight release in the airplane. Contributing factors include late flight, holding for connects, 'late' PA's, last min maintenance.

Title: FLC OF AN MLG DISCOVERED THAT THE LATEST MEL DEFERRED EQUIP ITEM WAS NOT INDICATED ON THE FLT RELEASE.
Narrative: DURING PREFLT CHKS WE NOTICED AN INOP LOOP 'A' ON THE #2 ENG. MAINT WAS CALLED OUT AND THE ITEM WAS PUT ON MEL. ALTHOUGH DISPATCH WAS NOTIFIED BY MAINT CTL OF THE ADDITION OF THE MEL, THERE WAS NO COM BTWN THE FLT DECK AND DISPATCH. THEREFORE THE APPROPRIATE NOTATION WAS NEVER ADDED TO THE FLT RELEASE IN THE AIRPLANE. CONTRIBUTING FACTORS INCLUDE LATE FLT, HOLDING FOR CONNECTS, 'LATE' PA'S, LAST MIN MAINT.
