Narrative:
On oct/xx/97 at XXX airport, between the hours of XA00 and XD00, I, xyz, svced all 4 engine oils on an air carrier B747, aircraft #YYY. I performed the engine oil servicing at the reservoirs, per air carrier work card instructions. Completing the work on 1 engine at a time, I removed the reservoir cap, inspected the oil level and then secured the cap. Although no oil needed to be added to any of the engines, the service was performed per air carrier work card and all caps were reinstalled and locked. The following day, oct/xz/97 the aircraft departed for destination. The pilot reported low oil pressure and shut down the #2 engine. The aircraft returned to airport. Upon inspection of the #2 engine, the oil cap was found not installed.

Title: A B747 AFTER TKOF SHUT DOWN #2 ENG AND RETURNED TO THE FIELD DUE TO LOSS OF OIL PRESSURE CAUSED BY THE OIL FILLER CAP NOT INSTALLED.
Narrative: ON OCT/XX/97 AT XXX ARPT, BTWN THE HRS OF XA00 AND XD00, I, XYZ, SVCED ALL 4 ENG OILS ON AN ACR B747, ACFT #YYY. I PERFORMED THE ENG OIL SVCING AT THE RESERVOIRS, PER ACR WORK CARD INSTRUCTIONS. COMPLETING THE WORK ON 1 ENG AT A TIME, I REMOVED THE RESERVOIR CAP, INSPECTED THE OIL LEVEL AND THEN SECURED THE CAP. ALTHOUGH NO OIL NEEDED TO BE ADDED TO ANY OF THE ENGS, THE SVC WAS PERFORMED PER ACR WORK CARD AND ALL CAPS WERE REINSTALLED AND LOCKED. THE FOLLOWING DAY, OCT/XZ/97 THE ACFT DEPARTED FOR DESTINATION. THE PLT RPTED LOW OIL PRESSURE AND SHUT DOWN THE #2 ENG. THE ACFT RETURNED TO ARPT. UPON INSPECTION OF THE #2 ENG, THE OIL CAP WAS FOUND NOT INSTALLED.
