Narrative:
Started takeoff and got confign warning. We failed to extend flaps to takeoff position and also failed to do checklist for takeoff. Took delay at gate for loading bags and received the weight and balance on ACARS while waiting. We set speed bugs, talked about using V1 minimum and stabilizer trim was at trim setting called for. We push back, started engines and taxied to runway 10C which took about 1/2 hour. I made 2 updates to passenger. We had little talk between us as this was the first time we flew together. Cleared into position and did line items on before takeoff checklist. Held in position for 1 or 2 mins, then started takeoff and got warning. We only moved a couple hundred ft, stopped and extended flaps. Did the before takeoff checklist and made takeoff. We had not passed the displaced threshold marking for runway 10C. I believe that receiving the weight and balance at gate instead of when taxiing that this made me think we had done the before takeoff checklist to the line. Possibly the same for the first officer as he would have extended flaps on start of taxi. All checklists were accomplished as called for except the before takeoff to the line.
Original NASA ASRS Text
Title: FLC OF B757 HAS CONFIGN WARNING ON TKOF AS A RESULT OF FLAPS NOT EXTENDED. TKOF ABORTED.
Narrative: STARTED TKOF AND GOT CONFIGN WARNING. WE FAILED TO EXTEND FLAPS TO TKOF POS AND ALSO FAILED TO DO CHKLIST FOR TKOF. TOOK DELAY AT GATE FOR LOADING BAGS AND RECEIVED THE WT AND BAL ON ACARS WHILE WAITING. WE SET SPD BUGS, TALKED ABOUT USING V1 MINIMUM AND STABILIZER TRIM WAS AT TRIM SETTING CALLED FOR. WE PUSH BACK, STARTED ENGS AND TAXIED TO RWY 10C WHICH TOOK ABOUT 1/2 HR. I MADE 2 UPDATES TO PAX. WE HAD LITTLE TALK BTWN US AS THIS WAS THE FIRST TIME WE FLEW TOGETHER. CLRED INTO POS AND DID LINE ITEMS ON BEFORE TKOF CHKLIST. HELD IN POS FOR 1 OR 2 MINS, THEN STARTED TKOF AND GOT WARNING. WE ONLY MOVED A COUPLE HUNDRED FT, STOPPED AND EXTENDED FLAPS. DID THE BEFORE TKOF CHKLIST AND MADE TKOF. WE HAD NOT PASSED THE DISPLACED THRESHOLD MARKING FOR RWY 10C. I BELIEVE THAT RECEIVING THE WT AND BAL AT GATE INSTEAD OF WHEN TAXIING THAT THIS MADE ME THINK WE HAD DONE THE BEFORE TKOF CHKLIST TO THE LINE. POSSIBLY THE SAME FOR THE FO AS HE WOULD HAVE EXTENDED FLAPS ON START OF TAXI. ALL CHKLISTS WERE ACCOMPLISHED AS CALLED FOR EXCEPT THE BEFORE TKOF TO THE LINE.
