Narrative:

We received the clearance on the ground in gtr to aex. The clearance is as filed. The clearance is igb VOR J52 sidon. The captain was flying and we made a right turn off of runway 18 to join J52 which is the 266 degree radial off of igb. The controller questioned why we were not proceeding to the VOR first. I didn't know that we were expected to do so. I have done this trip many times in the past 2 yrs. I have always made the right turn off the airport and have never once been questioned on it. This clearance was unclr to us and we didn't at all realize we were doing anything wrong.
