On jan/xx/98, quality assurance director xyz requested that myself and another inspector sign a duplicate copy of form, engineering order YYYY. This particular 'engineering order' had been accomplished in september of 1997, but for some unknown reason, quality assurance management needed a duplicate signed by us. Quality assurance director xyz said 'I discussed this with abc of FAA and it's okay with the FAA.' we then signed a document (engineering order YYYY) that was marked 'duplicate.'

Title: A DC9-50 WAS DISPATCHED AND FLOWN WITH A CARGO DOOR ENGINEERING ORDER ACCOMPLISHED WITHOUT THE PROPER DOCUMENTATION.

Narrative: ON JAN/XX/98, QUALITY ASSURANCE DIRECTOR XYZ REQUESTED THAT MYSELF AND ANOTHER INSPECTOR SIGN A DUPLICATE COPY OF FORM, ENGINEERING ORDER YYYY. THIS PARTICULAR 'ENGINEERING ORDER' HAD BEEN ACCOMPLISHED IN SEPTEMBER OF 1997, BUT FOR SOME UNKNOWN REASON, QUALITY ASSURANCE MGMNT NEEDED A DUPLICATE SIGNED BY US. QUALITY ASSURANCE DIRECTOR XYZ SAID 'I DISCUSSED THIS WITH ABC OF FAA AND IT'S OKAY WITH THE FAA.' WE THEN SIGNED A DOCUMENT (ENGINEERING ORDER YYYY) THAT WAS MARKED 'DUPLICATE.'
