Narrative:
During SID departure, captain said turn left. I didn't hear ATC give heading. Our departure was runway heading for vectors. I thought I didn't hear ATC vector, so I started gentle turn to left. I think my hesitancy to make turn caused captain to inform ATC of turn. Controller asked why we were turning, as SID was for runway heading. Captain had SID turned over, and was reading wrong SID. We quickly returned to runway heading, with very little deviation from SID. Supplemental information from acn 391584: selected wrong SID and initiated wrong turn out. Several distractions: 1) first time out of sjc. 2) IOE in progress. 3) runway and SID change late in preflight. 4) WX a factor. 5) SID page fell off clip and was turned upside down when replaced. The danville one departure was assigned off runway 12R. Plate fell off captain's clip and inadvertently the altam six departure was placed upside. At 4000 ft I called departure control with our intention of a turn to 300 degrees (incorrect). We were corrected without any conflict. Too many distractions at departure.

Title: A B757 DEPARTING SJC, CA, HAS A HEADING DEV BECAUSE THE CAPT DROPS THE SID PLATE ON THE COCKPIT FLOOR.
Narrative: DURING SID DEP, CAPT SAID TURN L. I DIDN'T HEAR ATC GIVE HEADING. OUR DEP WAS RWY HEADING FOR VECTORS. I THOUGHT I DIDN'T HEAR ATC VECTOR, SO I STARTED GENTLE TURN TO L.
