Narrative:
The situation in question concerns a repairman (xyyz) under the repair station who mistakenly tagged a part with an FAA 8130-3 (see attached) which was not on the capabilities listing of the repair station. This situation was discovered during an internal audit inspection on feb/xa/98. Upon the discovery the FAA was called immediately and appointment was scheduled for feb/xb/98 to discuss the problem. The switch part number 23374-4 in question was sent to the repair station by a customer under a work order and was accompanied by a certificate of conformance. The repair station was requested to place a protective coating on the outside of the switch (which does not change form, fit or function of the switch) in accordance with the airframe manufacturer's service bulletin 6/324 (see attached). This was accomplished during the period of may/95 through jul/95. This was accomplished when the repair station had an employee who was a licensed a&P mechanic. Initially the work requested by the customer was accomplished and the part was signed off by the licensed a&P mechanic working for the repair stations. After this mechanic left the company this work continued. Remaining units were completed and returned to the customer with an FAA 8130-3 signed off by the above named. It was the impression of this repairman that since the initial work was signed off by the a&P mechanic with the FAA 8130-3 form that the remaining units should be completed in the same manner. Upon the discovery of the error the customer was notified concerning the problem with the documentation and all inventory and paperwork related to this issue was returned and placed in quarantine at the repair station. The repair station has requested that this item be added to the capabilities listing. Upon the approval of this item on the capabilities listing of the repair station it is the intention to inspect and test the units and return them to the customer with the proper documentation. To preclude similar instances in the future the repair station has revised their internal work order requiring the mechanic or repairman to check the capabilities listing before accomplish any work (see attached). The FAA order 8130.21B dated nov/xx/97, has been reviewed by all repair station personnel and will be required reading for all new personnel.

Title: A DEHAVILLAND DHC6 SVC BULLETIN WAS ACCOMPLISHED BY A REPAIR STATION NOT ON THE FAA APPROVED LIST.
Narrative: THE SIT IN QUESTION CONCERNS A REPAIRMAN (XYYZ) UNDER THE REPAIR STATION WHO MISTAKENLY TAGGED A PART WITH AN FAA 8130-3 (SEE ATTACHED) WHICH WAS NOT ON THE CAPABILITIES LISTING OF THE REPAIR STATION. IT WAS THE IMPRESSION OF THIS REPAIRMAN THAT SINCE THE INITIAL WORK WAS SIGNED OFF BY THE A&P MECH WITH THE FAA 8130-3 FORM THAT THE REMAINING UNITS SHOULD BE COMPLETED IN THE SAME MANNER. UPON THE DISCOVERY OF THE ERROR THE CUSTOMER WAS NOTIFIED CONCERNING THE PROB WITH THE DOCUMENTATION AND ALL INVENTORY AND PAPERWORK RELATED TO THIS ISSUE WAS RETURNED AND PLACED IN QUARANTINE AT THE REPAIR STATION. THE REPAIR STATION HAS REQUESTED THAT THIS ITEM BE ADDED TO THE CAPABILITIES LISTING. UPON THE APPROVAL OF THIS ITEM ON THE CAPABILITIES LISTING OF THE REPAIR STATION IT IS THE INTENTION TO INSPECT AND TEST THE UNITS AND RETURN THEM TO THE CUSTOMER WITH THE PROPER DOCUMENTATION. TO PRECLUDE SIMILAR INSTANCES IN THE FUTURE THE REPAIR STATION HAS REVISED THEIR INTERNAL WORK ORDER REQUIRING THE MECH OR REPAIRMAN TO CHK THE CAPABILITIES LISTING BEFORE ACCOMPLISH ANY WORK (SEE ATTACHED). THE FAA ORDER 8130.21B DATED NOV/XX/97, HAS BEEN REVIEWED BY ALL REPAIR STATION PERSONNEL AND WILL BE REQUIRED READING FOR ALL NEW PERSONNEL.
