Narrative:
Shortly after leveling off, the failure occurred in the right engine, resulting in rough running. A precautionary shutdown of the engine was performed. The remaining flight was uneventful and a safe landing was made. Investigation revealed that the cylinder head on #4 cylinder had separated from the barrel completely. The cylinder remained in place. Engine oil was not contaminated. Airworthiness directive 78-08-07 applies to this condition. I suggest inspection for blow by before each flight instead of the 100 hour interval as per airworthiness directive. Note: the engine was installed 69 hours ago. It had undergone a complete overhaul.

Title: SMT ACFT OPERATING AS AIR TAXI IN CRUISE IN MICRONESIA HAD R ENG FAILURE DUE TO #4 CYLINDER HEAD SEPARATION FROM BARREL.
Narrative: SHORTLY AFTER LEVELING OFF, THE FAILURE OCCURRED IN THE R ENG, RESULTING IN ROUGH RUNNING. A PRECAUTIONARY SHUTDOWN OF THE ENG WAS PERFORMED. THE REMAINING FLT WAS UNEVENTFUL AND A SAFE LNDG WAS MADE. INVESTIGATION REVEALED THAT THE CYLINDER HEAD ON #4 CYLINDER HAD SEPARATED FROM THE BARREL COMPLETELY. THE CYLINDER REMAINED IN PLACE. ENG OIL WAS NOT CONTAMINATED. AIRWORTHINESS DIRECTIVE 78-08-07 APPLIES TO THIS CONDITION. I SUGGEST INSPECTION FOR BLOW BY BEFORE EACH FLT INSTEAD OF THE 100 HR INTERVAL AS PER AIRWORTHINESS DIRECTIVE. NOTE: THE ENG WAS INSTALLED 69 HRS AGO. IT HAD UNDERGONE A COMPLETE OVERHAUL.
