Narrative:

Up at XA00 for an XB00 crew bus pickup. 1 hour ride to airport. Checked in at operations and arrived at aircraft at XC15. Blocked out at XE06. During control check, while taxiing, ddpi showed no movement of left outboard aileron. Changed flap position and tried several more times. Visual check confirmed no movement of left outboard aileron. Returned to gate at XE40. Mechanics removed and replaced left outboard aileron lockout mechanism. This had been written up previously as erratic indication. I do not believe that a visual check was made. Aircraft may have been flown with aileron locked out. Blocked out again at XI38 for 8 hour trip to dusseldorf. Flight normal until passing 15000 ft MSL on descent. Flight engineer states #4 hydraulic system low fluid level. Suggests lowering gear early on approach. All agree. Passing 10000 ft landing lights turned on. 2 seconds later, heavy, white, acrid smoke coming from overhead panel. Electrical smoke/fire checklist performed. Landing lights turned off immediately. Smoke quits. Identify of source of smoke noted. #3 N1 gauge fails in descent. High RPM and speed brakes used to maintain anti-icing and descent profile to ILS. Copilot's electric altimeter fails at 5680 ft. Noted at 3000 ft FAF leveloff. Successful approach and landing made to minimum length wet runway. Cleared active and taxied to stand. Flight engineer makes 'states' power xfer to APU. On stand shutdown of #4 engine drops all lights in aircraft. #1 engine left running. APU indications zero. APU automatic shutdown during taxi-in apparent. Ground power connected. Shutdown complete. Crew kept at aircraft for noise. No copilot altimeter available. Duty day after 1 hour limousine to hotel was 19 hours. All these failures had maintenance write-ups previously. Mechanics writing off/deferring without maintenance. CRM does work!
