Narrative:
On may xa I was assigned to work air carrier X aircraft #xyz flight #yyz. Aircraft was return back to gate during flight. Upon inspection, I found oil cap was not secure on #2 engine. Callback conversation with reporter revealed the following information: the reporter stated the reporter serviced #2 engine oil tank but cannot recall if the cap was secured after adding oil. The reporter said the FAA has contacted the reporter for a statement.

Title: A B747-SP RETURNED TO THE FIELD DUE TO LOSS OF #2 ENG OIL CAUSED BY THE OIL FILLER CAP NOT SECURED AFTER SERVICING.
Narrative: ON MAY XA I WAS ASSIGNED TO WORK ACR X ACFT #XYZ FLT #YYZ. ACFT WAS RETURN BACK TO GATE DURING FLT. UPON INSPECTION, I FOUND OIL CAP WAS NOT SECURE ON #2 ENG. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: THE RPTR STATED THE RPTR SERVICED #2 ENG OIL TANK BUT CANNOT RECALL IF THE CAP WAS SECURED AFTER ADDING OIL. THE RPTR SAID THE FAA HAS CONTACTED THE RPTR FOR A STATEMENT.
