Narrative:
Aircraft on ground during routine pushback. Push was almost completed. Aircraft was brought to a stop but the headset operator had not yet asked for parking brake (as procedure requires). He then seemed to be doing something with the towbar near the nose. The tow driver then got out of the tug and also moved toward the nose. Shortly the tractor began to move and struck the aircraft on the radome. Just before impact the headset operator yelled 'set brake, set brake.' I set the brake just before impact. Aircraft radome was damaged and had to be replaced. Flight was canceled. Supplemental information from acn 404856: tug operator got out of the tug to help with the disconnect, although he did not set the parking brake on the tug, causing the tug to roll forward into the radome of the airplane.

Title: AN MD82 IS HIT IN THE RADOME BY A TUG AFTER THE PUSHBACK WAS COMPLETED. THE TUG MOVED FORWARD AFTER THE TUG DRIVER LEFT THE VEHICLE TO ASSIST THE OTHER CGP WITH THE TOWBAR DISCONNECT PROC.
Narrative: ACFT ON GND DURING ROUTINE PUSHBACK. PUSH WAS ALMOST COMPLETED. ACFT WAS BROUGHT TO A STOP BUT THE HEADSET OPERATOR HAD NOT YET ASKED FOR PARKING BRAKE (AS PROC REQUIRES). HE THEN SEEMED TO BE DOING SOMETHING WITH THE TOWBAR NEAR THE NOSE. THE TOW DRIVER THEN GOT OUT OF THE TUG AND ALSO MOVED TOWARD THE NOSE. SHORTLY THE TRACTOR BEGAN TO MOVE AND STRUCK THE ACFT ON THE RADOME. JUST BEFORE IMPACT THE HEADSET OPERATOR YELLED 'SET BRAKE, SET BRAKE.' I SET THE BRAKE JUST BEFORE IMPACT. ACFT RADOME WAS DAMAGED AND HAD TO BE REPLACED. FLT WAS CANCELED.
