Narrative:
Manley approach handled our flight from approximately 25 NM from the airport. Manley approach is not a radar equipped facility, therefore approach clearance time was given to us and updated as necessary for traffic separation. We were advised to hold at gudil intersection as published, which is at 4000 ft. After the third circuit, approach requested our position and I informed the controller we were approaching gudil. (First officer was the PF.) the controller immediately cleared us for the VOR DME runway 12 approach at approximately 1 mi or less from gudil. We failed to recognize the step-down to 2000 ft prior to crossing gudil and continued the approach from 4000 ft. The airport was VFR. About 5 mi on final a 360 degree turn was required in order to lose altitude and continue the approach at a normal sink rate and complete a safe landing. There were no other aircraft affected by our approach to runway 12.

Title: FLC ON B727 LNDG AT KINGSTON, JAMAICA, ENTERS A HOLDING PATTERN AT 4000 FT. AFTER BEING CLRED FOR THE VOR DME APCH, A 360 DEG TURN WAS REQUIRED ON FINAL TO LOSE ALT.
Narrative: MANLEY APCH HANDLED OUR FLT FROM APPROX 25 NM FROM THE ARPT. MANLEY APCH IS NOT A RADAR EQUIPPED FACILITY, THEREFORE APCH CLRNC TIME WAS GIVEN TO US AND UPDATED AS NECESSARY FOR TFC SEPARATION. WE WERE ADVISED TO HOLD AT GUDIL INTXN AS PUBLISHED, WHICH IS AT 4000 FT. AFTER THE THIRD CIRCUIT, APCH REQUESTED OUR POS AND I INFORMED THE CTLR WE WERE APCHING GUDIL. (FO WAS THE PF.) THE CTLR IMMEDIATELY CLRED US FOR THE VOR DME RWY 12 APCH AT APPROX 1 MI OR LESS FROM GUDIL. WE FAILED TO RECOGNIZE THE STEP-DOWN TO 2000 FT PRIOR TO XING GUDIL AND CONTINUED THE APCH FROM 4000 FT. THE ARPT WAS VFR. ABOUT 5 MI ON FINAL A 360 DEG TURN WAS REQUIRED IN ORDER TO LOSE ALT AND CONTINUE THE APCH AT A NORMAL SINK RATE AND COMPLETE A SAFE LNDG. THERE WERE NO OTHER ACFT AFFECTED BY OUR APCH TO RWY 12.
