Narrative:

On jul/xa/98, I worked my regular shift which is XA30 until XI30. The next day I worked the shift prior to my regular shift. The day was routine until the arrs and regular overnight layover aircraft started to arrival. We had a B757 which was coming in for a job to work a thrust reverser problem which was rework. When the B757 arrived we decided to do the check on the B757 so that next shift could focus on the thrust reverser problem. During the check we found a brake worn. While the other mechanics did the check, I decided to do the brake change. Another mechanic helped remove and put the brake on the axle. I had several interruptions during the installation process. I had to go inside to put the work package together for the next shift. After the work assignments were given out to the next shift, I returned and finished the brake. The aircraft flew for 3 days and on landing on the east coast the tower advised the captain of the B757 he had sparks on the right gear. On investigation they found the attach pin and bolt missing on the brake torsion bar. The pin and bolt had fallen out and allowed the bar to fall and drag on the ground.

Title: A B757-200 WAS DISPATCHED AND REMAINED IN SVC FOR 3 DAYS WITH AN IMPROPERLY INSTALLED MAIN GEAR BRAKE.

Narrative: ON JUL/XA/98, I WORKED MY REGULAR SHIFT WHICH IS XA30 UNTIL XI30. THE NEXT DAY I WORKED THE SHIFT PRIOR TO MY REGULAR SHIFT. THE DAY WAS ROUTINE UNTIL THE ARRS AND REGULAR OVERNIGHT LAYOVER ACFT STARTED TO ARR. WE HAD A B757 WHICH WAS COMING IN FOR A JOB TO WORK A THRUST REVERSER PROB WHICH WAS REWORK. WHEN THE B757 ARRIVED WE DECIDED TO DO THE CHK ON THE B757 SO THAT NEXT SHIFT COULD FOCUS ON THE THRUST REVERSER PROB. DURING THE CHK WE FOUND A BRAKE WORN. WHILE THE OTHER MECHS DID THE CHK, I DECIDED TO DO THE BRAKE CHANGE. ANOTHER MECH HELPED REMOVE AND PUT THE BRAKE ON THE AXLE. THE ACFT FLEW FOR 3 DAYS AND ON LNDG ON THE E COAST THE TWR ADVISED THE CAPT OF THE B757 HE HAD SPARKS ON THE R GEAR. ON INVESTIGATION THEY FOUND THE ATTACH PIN AND BOLT MISSING ON THE BRAKE TORSION BAR. THE PIN AND BOLT HAD FALLEN OUT AND ALLOWED THE BAR TO FALL AND DRAG ON THE GND.
