Narrative:

On contacting departure 120.15 they instructed us to turn heading 280 degrees at 5000 ft. Crew agreed this canceled our departure turn at 2 DME. Proceeded on heading we had at 1.5 DME. Passing through 4500 ft, departure asked if we were in the turn for the departure. PNF captain said no, must be misunderstanding. Recommendation: if clearance included words 'continue departure, then at 5000 ft turn 280 degrees,' would have understood, or we should have clarified. High workload at that point.
