Narrative:

Aircraft departed forbes field. I thought I departed the aircraft in computer but failed to do so, as I was interrupted by a call on the landline. Aircraft called field in sight at mci and I shipped him to their frequency. I began to suspect a problem so I looked for the strip and called approach to ask situation. They said there was a slight problem (later revealed as a supervisor seated at the sector position). I thought it was a safety issue. I then located the strip, and the flight plan in machine said it had not departed yet. I called back to approach to explain and they told me they were investigating because of the supervisor. I then told my supervisor there was a problem and was relieved from sector.

Title: ARTCC RADAR CTLR ALLOWS A BE90, DEPARTING FROM AN ADJOINING ARPT, TO PENETRATE CLASS B AIRSPACE ENRTE TO ANOTHER ARPT WITHOUT A HDOF. CTLR BECAME DISTR AND DID NOT COMPUTER DEPART THE ACFT. THE ACFT'S DATA TAG, THEREFORE, DID NOT AUTO ACQUIRE AND WAS NOT HANDED OFF TO THE ADJOINING APCH CTLR.

Narrative: ACFT DEPARTED FORBES FIELD. I THOUGHT I DEPARTED THE ACFT IN COMPUTER BUT FAILED TO DO SO, AS I WAS INTERRUPTED BY A CALL ON THE LANDLINE. ACFT CALLED FIELD IN SIGHT AT MCI AND I SHIPPED HIM TO THEIR FREQ. I BEGAN TO SUSPECT A PROB SO I LOOKED FOR THE STRIP AND CALLED APCH TO ASK SIT. THEY SAID THERE WAS A SLIGHT PROB (LATER REVEALED AS A SUPVR SEATED AT THE SECTOR POS). I THOUGHT IT WAS A SAFETY ISSUE. I THEN LOCATED THE STRIP, AND THE FLT PLAN IN MACHINE SAID IT HAD NOT DEPARTED YET. I CALLED BACK TO APCH TO EXPLAIN AND THEY TOLD ME THEY WERE INVESTIGATING BECAUSE OF THE SUPVR. I THEN TOLD MY SUPVR THERE WAS A PROB AND WAS RELIEVED FROM SECTOR.
