Cleared for approach to runway 5R (ILS). Failed to switch from approach control to tower frequency. Landed. Noticed frequency (wrong one) in communication panel when contacting ground control on exiting runway. Contributors: fatigue -- first officer on 5TH day of flying after 'minimum overnight' with inadequate sleep.
