Narrative:
When #1 for departure on runway 28L at pit, the first officer began to make a PA informing the folks we were next for departure. At the exact same time, pit tower cleared us 'into position and hold.' a BE1900 was stationary on the runway having already been cleared for takeoff. We waited for the BE1900 to begin its roll and then taxied into position. The runway checklist was performed and the first officer advanced the power for takeoff. Takeoff was completed and we climbed out of 400 ft AGL when pit tower asked us what we were doing. I still had no idea what we had done until informed by tower. The PA announcement was made at the appropriate time by the first officer. I was the crew member who heard and read back 'position and hold.' without stating to the first officer we were cleared for takeoff, I still gave every indication we were by completing the runway checklist and telling the first officer it was his/her airplane. We were fatigued. The first officer was temporarily out of the loop.

Title: FLC OF A DEHAVILLAND DASH 8, DH8, TOOK OFF WITHOUT CLRNC. THE ATCT LCL CTLR CALLED THEIR ATTN TO THE FACT PRIOR TO TURNING THEM OVER TO DEP CTL.
Narrative: WHEN #1 FOR DEP ON RWY 28L AT PIT, THE FO BEGAN TO MAKE A PA INFORMING THE FOLKS WE WERE NEXT FOR DEP. AT THE EXACT SAME TIME, PIT TWR CLRED US 'INTO POS AND HOLD.' A BE1900 WAS STATIONARY ON THE RWY HAVING ALREADY BEEN CLRED FOR TKOF. WE WAITED FOR THE BE1900 TO BEGIN ITS ROLL AND THEN TAXIED INTO POS. THE RWY CHKLIST WAS PERFORMED AND THE FO ADVANCED THE PWR FOR TKOF. TKOF WAS COMPLETED AND WE CLBED OUT OF 400 FT AGL WHEN PIT TWR ASKED US WHAT WE WERE DOING. I STILL HAD NO IDEA WHAT WE HAD DONE UNTIL INFORMED BY TWR. THE PA ANNOUNCEMENT WAS MADE AT THE APPROPRIATE TIME BY THE FO. I WAS THE CREW MEMBER WHO HEARD AND READ BACK 'POS AND HOLD.' WITHOUT STATING TO THE FO WE WERE CLRED FOR TKOF, I STILL GAVE EVERY INDICATION WE WERE BY COMPLETING THE RWY CHKLIST AND TELLING THE FO IT WAS HIS/HER AIRPLANE. WE WERE FATIGUED. THE FO WAS TEMPORARILY OUT OF THE LOOP.
