Narrative:
On wednesday I was issued job card xax-abc along with write-up stating that the main entrance door handle torque test had failed the 600 pound in torque limit, which is item #2 on job card #xax-abc. While operating door, I found that the top aft latch roller was not engaging its latch properly and made door hard to close. I believe the top aft roller being out of adjustment caused the problem of main entrance door handle torque test failure. I adjusted the roller to fit properly into its latch by adjusting the programming arm. Found door to be within torque handle limits so I signed off the write-up. On thursday I was informed that the main entrance door handle on aircraft bzb had unlatched during pressurization while taking off from XXX and again at depressurization coming into YYY. Both times the stewardess closed the door handle. Earlier in the day aircraft bzb had flown from ZZZ to XXX without incident.

Title: A B737-400 ON 2 SEPARATE OCCASIONS ON INITIAL CABIN PRESSURIZATION THE FORWARD CABIN DOOR HANDLE ROTATED UP FROM THE CLOSED AND LOCKED POS.
Narrative: ON WEDNESDAY I WAS ISSUED JOB CARD XAX-ABC ALONG WITH WRITE-UP STATING THAT THE MAIN ENTRANCE DOOR HANDLE TORQUE TEST HAD FAILED THE 600 LB IN TORQUE LIMIT, WHICH IS ITEM #2 ON JOB CARD #XAX-ABC. WHILE OPERATING DOOR, I FOUND THAT THE TOP AFT LATCH ROLLER WAS NOT ENGAGING ITS LATCH PROPERLY AND MADE DOOR HARD TO CLOSE. I BELIEVE THE TOP AFT ROLLER BEING OUT OF ADJUSTMENT CAUSED THE PROB OF MAIN ENTRANCE DOOR HANDLE TORQUE TEST FAILURE. I ADJUSTED THE ROLLER TO FIT PROPERLY INTO ITS LATCH BY ADJUSTING THE PROGRAMMING ARM. FOUND DOOR TO BE WITHIN TORQUE HANDLE LIMITS SO I SIGNED OFF THE WRITE-UP. ON THURSDAY I WAS INFORMED THAT THE MAIN ENTRANCE DOOR HANDLE ON ACFT BZB HAD UNLATCHED DURING PRESSURIZATION WHILE TAKING OFF FROM XXX AND AGAIN AT DEPRESSURIZATION COMING INTO YYY. BOTH TIMES THE STEWARDESS CLOSED THE DOOR HANDLE. EARLIER IN THE DAY ACFT BZB HAD FLOWN FROM ZZZ TO XXX WITHOUT INCIDENT.
