Narrative:
While descending into ord on the bradford 3 arrival, we were cleared to cross bdf at FL240. I screwed up and looked at the wrong DME. About 6 mi from bdf the center controller said that I wasn't going to make the restr and to increase rate of descent. I had been slowly descending so I pulled the power to idle and pulled out the speed brakes. Now I missed my turn over bdf and the next controller reminded me to turn. On final approach, my first officer had to remind me of final flaps. I have always preached that mistakes tend to snowball. Little mistakes lead to much bigger mistakes and one should slow down, gather one's thoughts and regroup. I had my head up my ass the entire flight! I was thoroughly disgusted with myself and vowed to not allow this to happen again.

Title: AN ACR PIC COMES TO TERMS WITH HIMSELF AFTER ALMOST MISSING AN ALT XING RESTR, MISSING THE TURN OVER BDF AND NOT EXTENDING FINAL FLAPS FOR LNDG. ALL WERE CORRECTED BY 2 CTLRS AND ONE FO.
Narrative: WHILE DSNDING INTO ORD ON THE BRADFORD 3 ARR, WE WERE CLRED TO CROSS BDF AT FL240. I SCREWED UP AND LOOKED AT THE WRONG DME. ABOUT 6 MI FROM BDF THE CTR CTLR SAID THAT I WASN'T GOING TO MAKE THE RESTR AND TO INCREASE RATE OF DSCNT. I HAD BEEN SLOWLY DSNDING SO I PULLED THE PWR TO IDLE AND PULLED OUT THE SPD BRAKES. NOW I MISSED MY TURN OVER BDF AND THE NEXT CTLR REMINDED ME TO TURN. ON FINAL APCH, MY FO HAD TO REMIND ME OF FINAL FLAPS. I HAVE ALWAYS PREACHED THAT MISTAKES TEND TO SNOWBALL. LITTLE MISTAKES LEAD TO MUCH BIGGER MISTAKES AND ONE SHOULD SLOW DOWN, GATHER ONE'S THOUGHTS AND REGROUP. I HAD MY HEAD UP MY ASS THE ENTIRE FLT! I WAS THOROUGHLY DISGUSTED WITH MYSELF AND VOWED TO NOT ALLOW THIS TO HAPPEN AGAIN.
