Narrative:
Location: dhp 337 degree radial, approximately 15 DME. Briefed mia 1 departure. After turning 020 degrees to intercept the hedley transition (phk 157 degree radial), captured the 337 degree radial outbound dhp. Dhp was in the frequency window with VOR armed. Simultaneously realizing the incorrect frequency was entered, mia departure (119.45) told us to turn to 360 degrees to reintercept the phk 157 degree radial. Complied with instructions. In reviewing the mia 1 SID, the black line connecting phk and the mia airport (shaded area) misled us to have the dhp frequency initially entered. Having flown this SID several times in the past we just overlooked the correct NAVAID frequency. We will attempt to be more aware for future flying.
Original NASA ASRS Text
Title: FLC OF A SUPER MD80 FAILED TO FOLLOW THE PUBLISHED SID TRACK DURING CLB AFTER TKOF RESULTING IN ATC INTERVENING AND PROVIDING A HDG TO BRING THEM BACK TO CLRNC TRACK.
Narrative: LOCATION: DHP 337 DEG RADIAL, APPROX 15 DME. BRIEFED MIA 1 DEP. AFTER TURNING 020 DEGS TO INTERCEPT THE HEDLEY TRANSITION (PHK 157 DEG RADIAL), CAPTURED THE 337 DEG RADIAL OUTBOUND DHP. DHP WAS IN THE FREQ WINDOW WITH VOR ARMED. SIMULTANEOUSLY REALIZING THE INCORRECT FREQ WAS ENTERED, MIA DEP (119.45) TOLD US TO TURN TO 360 DEGS TO REINTERCEPT THE PHK 157 DEG RADIAL. COMPLIED WITH INSTRUCTIONS. IN REVIEWING THE MIA 1 SID, THE BLACK LINE CONNECTING PHK AND THE MIA ARPT (SHADED AREA) MISLED US TO HAVE THE DHP FREQ INITIALLY ENTERED. HAVING FLOWN THIS SID SEVERAL TIMES IN THE PAST WE JUST OVERLOOKED THE CORRECT NAVAID FREQ. WE WILL ATTEMPT TO BE MORE AWARE FOR FUTURE FLYING.
