Narrative:
On choshi 8 departure, first officer indicated I was closing on 094 degree radial hme and switched my radio to hme. In so doing, DME to/from nrt was lost. I initiated turn to intercept 094 degree radial hme prior to 12 DME from nrt, inadvertently taking me over noise sensitive area. Departure control then vectored me through 094 degree on 160 degree heading, telling me I was violating departure but not indicating why. 1) departure is poorly written and diagrammed. A) hme radial does not indicate proper path. B) should have 'turn on' path indicated to show rejoining 094 degree radial after 12 DME. 2) departure control unable to relate to problem in clear manner. 3) both so and first officer saw turn was early but failed to indicate same to me. Very poor input as CRM was stressed to crew prior to departure on first leg of trip. 4) I was preoccupied with takeoff performance of aircraft, ie, unexplained veer to right during final phase of takeoff roll and very sluggish climb profile.
