Air Traffic Incident | Pilot Deviation |
Narrative:
Flight plan was to intercept V-45 (approximately 201 degree radial from apn). I was picking up a new plane, with avionics I haven't used for 8 yrs (HSI). When setting obs I must have twisted the compass card. I ended up flying approximately 45 degree radial from apn. The audio panel had intermittent switches and communications were temporarily lost. When communications were recovered tower advised of course deviation (significant) and altitude busted (600 ft). Having figured the problem out, HSI card was reset to compass. Tower gave me vector to V-45 and inadvertent altitude bust was corrected, and the proper airway was joined. There were no other traffic so there was no separation issues. Unfamiliarity with new plane, new instrument panel, new type of communication/navigation/radios and IMC flight rust all contributed. The errors were discovered and corrected. Remaining trip was without trouble.

Title: THE RPTR WAS FLYING A NEW PA28A THAT HAD AN AVIONICS SYS OF WHICH HE WAS UNFAMILIAR AND THE COM SYS WAS INTERMITTENT. HE MISSET THE NAV SYS, LOST COM TEMPORARILY, GOT OFF COURSE AND EXCEEDED HIS ASSIGNED ALT.
Narrative: FLT PLAN WAS TO INTERCEPT V-45 (APPROX 201 DEG RADIAL FROM APN). I WAS PICKING UP A NEW PLANE, WITH AVIONICS I HAVEN'T USED FOR 8 YRS (HSI). WHEN SETTING OBS I MUST HAVE TWISTED THE COMPASS CARD. I ENDED UP FLYING APPROX 45 DEG RADIAL FROM APN. THE AUDIO PANEL HAD INTERMITTENT SWITCHES AND COMS WERE TEMPORARILY LOST. WHEN COMS WERE RECOVERED TWR ADVISED OF COURSE DEV (SIGNIFICANT) AND ALT BUSTED (600 FT). HAVING FIGURED THE PROB OUT, HSI CARD WAS RESET TO COMPASS. TWR GAVE ME VECTOR TO V-45 AND INADVERTENT ALT BUST WAS CORRECTED, AND THE PROPER AIRWAY WAS JOINED. THERE WERE NO OTHER TFC SO THERE WAS NO SEPARATION ISSUES. UNFAMILIARITY WITH NEW PLANE, NEW INST PANEL, NEW TYPE OF COM/NAV/RADIOS AND IMC FLT RUST ALL CONTRIBUTED. THE ERRORS WERE DISCOVERED AND CORRECTED. REMAINING TRIP WAS WITHOUT TROUBLE.
