Narrative:
Aircraft #1 was cleared from ill to msp to maintain 5000 ft through flight service. Aircraft #1 checked on frequency departing the airport. I told him to report reaching 5000 ft. He never reported 5000 ft or turned on his transponder. He made it all the way to msp airport before calling again. Mistakes I made, one, I departed the aircraft in the computer but didn't write a time in box 22 of strips. Two, I did not manually start a track on aircraft. Three, I also took a break a few mins after he departed and forgot to tell the relieving controller he had departed.

Title: ZMP CTLR ISSUES CLRNC TO IFR PA31 FROM SATELLITE MSP ARPT WITHOUT RADAR IDENTING THE ACFT. CTLR IS RELIEVED, NOT PROVIDING A COMPLETE SECTOR BRIEFING, AND THE ACFT PENETRATES MSP CLASS B WITHOUT A HDOF.
Narrative: ACFT #1 WAS CLRED FROM ILL TO MSP TO MAINTAIN 5000 FT THROUGH FLT SVC. ACFT #1 CHKED ON FREQ DEPARTING THE ARPT. I TOLD HIM TO RPT REACHING 5000 FT. HE NEVER RPTED 5000 FT OR TURNED ON HIS XPONDER. HE MADE IT ALL THE WAY TO MSP ARPT BEFORE CALLING AGAIN. MISTAKES I MADE, ONE, I DEPARTED THE ACFT IN THE COMPUTER BUT DIDN'T WRITE A TIME IN BOX 22 OF STRIPS. TWO, I DID NOT MANUALLY START A TRACK ON ACFT. THREE, I ALSO TOOK A BREAK A FEW MINS AFTER HE DEPARTED AND FORGOT TO TELL THE RELIEVING CTLR HE HAD DEPARTED.
