Narrative:

Due to a clerical error on my part, my medical expired. I have my due dates written down. My flight instructor is up 6/99, my medical was up 9/98. I had written down 9/99 for the medical and thus let the medical lapse. I have not been flying full time limited flight instructions and I believe everyone I flew with during the expired time were qualified pilots, so I didn't violate the FARS, but it's a big oops anyway. Not flying full time and clerical error led to this.

Title: AIRLINE TRANSPORT RATED PLT INADVERTENTLY FAILED TO RENEW HIS AIRMAN'S MEDICAL CERTIFICATE DUE TO CONFUSING THE EXPIRATION DATE OF IT WITH HIS FLT INSTRUCTOR CERTIFICATE.

Narrative: DUE TO A CLERICAL ERROR ON MY PART, MY MEDICAL EXPIRED. I HAVE MY DUE DATES WRITTEN DOWN. MY FLT INSTRUCTOR IS UP 6/99, MY MEDICAL WAS UP 9/98. I HAD WRITTEN DOWN 9/99 FOR THE MEDICAL AND THUS LET THE MEDICAL LAPSE. I HAVE NOT BEEN FLYING FULL TIME LIMITED FLT INSTRUCTIONS AND I BELIEVE EVERYONE I FLEW WITH DURING THE EXPIRED TIME WERE QUALIFIED PLTS, SO I DIDN'T VIOLATE THE FARS, BUT IT'S A BIG OOPS ANYWAY. NOT FLYING FULL TIME AND CLERICAL ERROR LED TO THIS.
