Narrative:
Aircraft filled up with smoke while at the gate at lax with passenger on board. The passenger were evacuate/evacuationed in an orderly manner out the front door. Maintenance was called. It was determined that APU oil was contaminating the air conditioning packs. The APU and aircraft were released for dispatch. The passenger were reboarded. A normal takeoff ensued, however, at approximately 3000 ft MSL the flight attendants informed the crew that smoke was filling up the cabin. Both air conditioning packs were turned off by the crew and the outflow valve for the cabin was manually opened. The smoke subsided. The aircraft immediately returned to lax for a normal landing. The aircraft returned to the gate where all of the passenger deplaned in a normal manner. There were no injuries. I feel that maintenance and flight control should be more thorough in their evaluation of an aircraft before returning it to service.
Original NASA ASRS Text
Title: AN ACR FLC FLYING A 117 SEAT COMMERCIAL FIXED WING TURBOJET EXPERIENCED SMOKE IN THE CABIN WHILE AT THE GATE AT LAX. IT WAS LATER DETERMINED THAT APU OIL WAS CONTAMINATING THE AIR CONDITIONING SYS AND THE ACFT WAS RELEASED FOR SVC. AFTER TKOF THE CABIN AGAIN FILLED WITH SMOKE AND THE FLC RETURNED TO LAX.
Narrative: ACFT FILLED UP WITH SMOKE WHILE AT THE GATE AT LAX WITH PAX ON BOARD. THE PAX WERE EVACED IN AN ORDERLY MANNER OUT THE FRONT DOOR. MAINT WAS CALLED. IT WAS DETERMINED THAT APU OIL WAS CONTAMINATING THE AIR CONDITIONING PACKS. THE APU AND ACFT WERE RELEASED FOR DISPATCH. THE PAX WERE REBOARDED. A NORMAL TKOF ENSUED, HOWEVER, AT APPROX 3000 FT MSL THE FLT ATTENDANTS INFORMED THE CREW THAT SMOKE WAS FILLING UP THE CABIN. BOTH AIR CONDITIONING PACKS WERE TURNED OFF BY THE CREW AND THE OUTFLOW VALVE FOR THE CABIN WAS MANUALLY OPENED. THE SMOKE SUBSIDED. THE ACFT IMMEDIATELY RETURNED TO LAX FOR A NORMAL LNDG. THE ACFT RETURNED TO THE GATE WHERE ALL OF THE PAX DEPLANED IN A NORMAL MANNER. THERE WERE NO INJURIES. I FEEL THAT MAINT AND FLT CTL SHOULD BE MORE THOROUGH IN THEIR EVALUATION OF AN ACFT BEFORE RETURNING IT TO SVC.
