Narrative:

During pushback, the aft flight attendant rang the cockpit. Captain was monitoring the push crewman, and I was monitoring the ramp controller per SOP. Captain answered the aft flight attendant and got into a discussion with her about a noise. After the start of the right engine, the mechanic backed the tug up to release the tow bar. At that time, the captain and I noticed the aircraft rolling forward and both applied the brakes. The mechanic re-established communications with the captain, and informed him that he inspected the airplane and found no damage, but the tow bar handle was sheared off. Captain said he did not hear the mechanic tell him to set the parking brake during his distraction with the flight attendant.

Title: A B757 FLC DID NOT SET THE PARKING BRAKE AFTER PUSHBACK AT DEN, AND ROLLED FORWARD DAMAGING THE TOW BAR.

Narrative: DURING PUSHBACK, THE AFT FLT ATTENDANT RANG THE COCKPIT. CAPT WAS MONITORING THE PUSH CREWMAN, AND I WAS MONITORING THE RAMP CTLR PER SOP. CAPT ANSWERED THE AFT FLT ATTENDANT AND GOT INTO A DISCUSSION WITH HER ABOUT A NOISE. AFTER THE START OF THE R ENG, THE MECH BACKED THE TUG UP TO RELEASE THE TOW BAR. AT THAT TIME, THE CAPT AND I NOTICED THE ACFT ROLLING FORWARD AND BOTH APPLIED THE BRAKES. THE MECH RE-ESTABLISHED COMS WITH THE CAPT, AND INFORMED HIM THAT HE INSPECTED THE AIRPLANE AND FOUND NO DAMAGE, BUT THE TOW BAR HANDLE WAS SHEARED OFF. CAPT SAID HE DID NOT HEAR THE MECH TELL HIM TO SET THE PARKING BRAKE DURING HIS DISTR WITH THE FLT ATTENDANT.
