Narrative:

On dec/tue/99, I flew ship #xyz to ord. I thought I had checked the paperwork carefully before, but I usually check the entire flight plan again in cruise. It was then I found ship #xzy listed on the flight plan. Apparently in the rush to push back I had transposed the numbers. In the future if the ship numbers are so close I suggest that one pilot read the ship number and the other pilot confirm that number.

Title: A BOEING 757 FLC DISCOVERED THE FLT PLAN, DISPATCH RELEASE FORMS AND THE ACFT BEING FLOWN DIFFERED AFTER DEP FROM ATL.

Narrative: ON DEC/TUE/99, I FLEW SHIP #XYZ TO ORD. I THOUGHT I HAD CHKED THE PAPERWORK CAREFULLY BEFORE, BUT I USUALLY CHK THE ENTIRE FLT PLAN AGAIN IN CRUISE. IT WAS THEN I FOUND SHIP #XZY LISTED ON THE FLT PLAN. APPARENTLY IN THE RUSH TO PUSH BACK I HAD TRANSPOSED THE NUMBERS. IN THE FUTURE IF THE SHIP NUMBERS ARE SO CLOSE I SUGGEST THAT ONE PLT READ THE SHIP NUMBER AND THE OTHER PLT CONFIRM THAT NUMBER.
