Narrative:

All preflight and before takeoff procedures were normal. While taking off and reaching vr, we heard loud compressor stalls and significant vibration on the right side of the aircraft. Tower reported fire from the right side. Company procedures were followed and a southeast landing at departure field was initiated. After an unsafe main indication was handled, an uneventful landing was made. It was determined that a recapped tire had destructed and fodded the engine. This is a continuing problem on our aircraft. Callback conversation with reporter revealed the following information: the reporter stated the entire recap did not peel off the tire carcass, only about a 6 inch strip in the center and around the entire tire was thrown off. The reporter stated the aircraft incurred damage on the main gear strut and gear position sensors, as well as the serious engine ingestion. The reporter said no information was obtained from maintenance as to how many recaps had been on this tire. The reporter is not aware of any company policy limiting the number of recaps allowed on a tire carcass.

Title: AN MD88 ON TKOF AT ROTATION DECLARED AN EMER AND DIVERTED DUE TO NUMBER 3 MAIN TIRE THROWN TREAD INGESTED INTO NUMBER 2 ENG AND CAUSING MAIN GEAR UNSAFE WARNING.

Narrative: ALL PREFLT AND BEFORE TKOF PROCS WERE NORMAL. WHILE TAKING OFF AND REACHING VR, WE HEARD LOUD COMPRESSOR STALLS AND SIGNIFICANT VIBRATION ON THE R SIDE OF THE ACFT. TWR RPTED FIRE FROM THE R SIDE. COMPANY PROCS WERE FOLLOWED AND A SE LNDG AT DEP FIELD WAS INITIATED. AFTER AN UNSAFE MAIN INDICATION WAS HANDLED, AN UNEVENTFUL LNDG WAS MADE. IT WAS DETERMINED THAT A RECAPPED TIRE HAD DESTRUCTED AND FODDED THE ENG. THIS IS A CONTINUING PROB ON OUR ACFT. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: THE RPTR STATED THE ENTIRE RECAP DID NOT PEEL OFF THE TIRE CARCASS, ONLY ABOUT A 6 INCH STRIP IN THE CTR AND AROUND THE ENTIRE TIRE WAS THROWN OFF. THE RPTR STATED THE ACFT INCURRED DAMAGE ON THE MAIN GEAR STRUT AND GEAR POS SENSORS, AS WELL AS THE SERIOUS ENG INGESTION. THE RPTR SAID NO INFO WAS OBTAINED FROM MAINT AS TO HOW MANY RECAPS HAD BEEN ON THIS TIRE. THE RPTR IS NOT AWARE OF ANY COMPANY POLICY LIMITING THE NUMBER OF RECAPS ALLOWED ON A TIRE CARCASS.
