Narrative:

A fuel sumping mechanic without airworthiness release signed his name and employee number under maintenance action in log page but left out employee number in blocks above blank since he had no airworthiness release. I, thinking he forgot to enter his employee number, entered his number in the blocks above, as I was closing out his paperwork. I was not intending to forge or incriminate him in any way. It was an honest mistake.

Title: A B737-500 WAS DISPATCHED IN NON COMPLIANCE WITH INCORRECT LOGBOOK ENTRIES BY AN UNQUALIFIED TECHNICIAN.

Narrative: A FUEL SUMPING MECH WITHOUT AIRWORTHINESS RELEASE SIGNED HIS NAME AND EMPLOYEE NUMBER UNDER MAINT ACTION IN LOG PAGE BUT LEFT OUT EMPLOYEE NUMBER IN BLOCKS ABOVE BLANK SINCE HE HAD NO AIRWORTHINESS RELEASE. I, THINKING HE FORGOT TO ENTER HIS EMPLOYEE NUMBER, ENTERED HIS NUMBER IN THE BLOCKS ABOVE, AS I WAS CLOSING OUT HIS PAPERWORK. I WAS NOT INTENDING TO FORGE OR INCRIMINATE HIM IN ANY WAY. IT WAS AN HONEST MISTAKE.
