Narrative:
Failed to place in logbook a maintenance item that stopped working. Far requires this be placed in logbook at time of failures and dispatch notified. This was an alternate start valve/ignition which we wrote up going into nrt. 'By the book' we should have entered into log -- I was unaware of this until it was brought up in discussion with another crew. Will go by FARS better in future.

Title: A B747-400 WAS DISPATCHED IN NON COMPLIANCE WITH A KNOWN MECHANICAL FAILURE NOT ENTERED IN THE ACFT FLT LOG.
Narrative: FAILED TO PLACE IN LOGBOOK A MAINT ITEM THAT STOPPED WORKING. FAR REQUIRES THIS BE PLACED IN LOGBOOK AT TIME OF FAILURES AND DISPATCH NOTIFIED. THIS WAS AN ALTERNATE START VALVE/IGNITION WHICH WE WROTE UP GOING INTO NRT.
