Narrative:
You can't make this stuff up. Last month we had an operational error where the amended call sign (the real one) was put in the remarks section. The controller couldn't get the aircraft to track, got distraction and smoked him with a departing learjet. The remarks section has become the catch all for anything a pilot or controller can't figure out how to put into the computer. P.south. I wrote this at dinner. Went back, and the strip was still on the boards at sector 46 and sector 48. So much for being overdue.
