Narrative:
We were to fly flight anc-fai. My release showed an MEL for autothrottles. The crew that brought in the airplane had written up the cabin rate of climb indicator. Maintenance put the indicator on MEL and the MEL book said it is not a required instrument as long as the pressurization system functions normally -- and it did. I needed to get an amendment adding this MEL to my release -- something I have done 1000 times. My first officer and I forgot to do this and we flew to fai without the amendment.

Title: A B737-400 FLC FLIES THEIR ACFT FROM ANC WITHOUT HAVING AN MEL AMENDMENT MADE TO THEIR DISPATCH RELEASE REGARDING AN INOP CABIN RATE OF CLB INDICATOR AT ANC, AK.
Narrative: WE WERE TO FLY FLT ANC-FAI. MY RELEASE SHOWED AN MEL FOR AUTOTHROTTLES. THE CREW THAT BROUGHT IN THE AIRPLANE HAD WRITTEN UP THE CABIN RATE OF CLB INDICATOR. MAINT PUT THE INDICATOR ON MEL AND THE MEL BOOK SAID IT IS NOT A REQUIRED INST AS LONG AS THE PRESSURIZATION SYS FUNCTIONS NORMALLY -- AND IT DID. I NEEDED TO GET AN AMENDMENT ADDING THIS MEL TO MY RELEASE -- SOMETHING I HAVE DONE 1000 TIMES. MY FO AND I FORGOT TO DO THIS AND WE FLEW TO FAI WITHOUT THE AMENDMENT.
