Narrative:

Location: manaus center. MD11. We had an air conditioning pack that had been placarded for several days. We discussed it with 2 mechanics and compiled with MEL procedures before we left the gate. While en route, we had a conversation with dispatch and maintenance when another pack became inoperative. Both dispatch and maintenance said they were not aware that the pack was placarded.

Narrative: LOCATION: MANAUS CTR. MD11. WE HAD AN AIR CONDITIONING PACK THAT HAD BEEN PLACARDED FOR SEVERAL DAYS. WE DISCUSSED IT WITH 2 MECHS AND COMPILED WITH MEL PROCS BEFORE WE LEFT THE GATE. WHILE ENRTE, WE HAD A CONVERSATION WITH DISPATCH AND MAINT WHEN ANOTHER PACK BECAME INOP. BOTH DISPATCH AND MAINT SAID THEY WERE NOT AWARE THAT THE PACK WAS PLACARDED.
